29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  (a)  On or about 2/14/2017, during residential construction activities, located at 6621 Wildwood Road, in Hudson Ohio, the employer failed to ensure that the scaffolding was on an adequate firm foundation. The scaffolding was propped up by a vertical standing 2"x 6" piece of lumber.  (b) On or about 2/14/2017, during residential construction activities, located at 6621 Wildwood Road, in Hudson Ohio, the employer failed to ensure that the scaffolding was erected with adequate cross bracing.   (c) On or about 2/14/2017, during residential construction activities, located at 6621 Wildwood Road, in Hudson Ohio, the employer failed to ensure that the employees working at a height greater than 11 feet were provided with a standard railing system.   (d) On or about 2/14/2017, during residential construction activities, located at 6621 Wildwood Road, in Hudson Ohio, the employer failed to ensure that the scaffolding was erected with a toeboard.   (e) On or about 2/14/2017, during residential construction activities located at 6621 Wildwood Road, in Hudson Ohio, the employer failed to ensure that the working levels of the scaffold were fully planked.  (f) On or about 2/14/2017, during residential construction activities, located at 6621 Wildwood Road, in Hudson Ohio, the employer failed to ensure that the employees were not using the cross bracing as a means of access.
