29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, 1441 Benton Road, Pittsburgh, PA: On or about February 15, 2017, during roofing activities, the employer did not ensure that the ladder, used to gain access to/egress from the roof of the house was properly set up in that it extended less than 1 foot above the roof line, it was not secured at it's top to prevent deflection and a grab rail was not provided.
