29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:   On or about at 2/16/17 located at the front entrance:  An employee was climbing the rails / edge of the areal lift to get up in to the window to exit the lift at approximately 40 - feet above the lower level and exposed to a fall hazard of up to 40  foot.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about at 2/16/17 located at the front entrance: The employer failed to enforce his employees to use their fall protection while installing the plywood siding and was exposed to a fall hazard of up to 40 - foot from an aerial lift to the lower level.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.   On or about at 2/16/17 located at the front entrance Employees were exposed to fall hazards, while framing a new hotel, employees were working at the windows and door openings that had no guards and exposed to approximately 40 - foot fall hazards.
