Building 4 - On or about February 15, 2017, employees were installing 2nd floor trusses while working from already installed floor trusses without a fall protection system in place, thus exposing employees to a fall hazard.

Building 4 - On or about February 15, 2017, employees used the top cap and top step of a Louisville (model: W-3215-06) and a Werner (model: FS206) step ladder to access/egress the 2nd floor trusses and walls, thus exposing employees to a fall hazard.
