Executool Precision Tooling, Inc., 2727 W. 16th St. Erie, PA, East wall of Mill room area, On or about February 17th, 2017 and times prior thereto:  Employees utilized a Roll-In brand vertical band saw (Model: EF1459; Serial # 95175EF) that was not equipped with a guard to cover the unused portion of the saw blade and the nip points created by the moving saw blade and blade guide rolls.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:           a.)  Executool Precision Tooling, Inc., 2727 W. 16th St. Erie, PA, Maintenance shop area, On or about February 17th, 2017 and time prior thereto:  Employees utilized a Dayton Bench grinder (Model # 1Z707W; Manufacturers Reference # 604477WB903) for various grinding operations onsite.  The Dayton bench grinder (left hand side) was not equipped with a properly functioning tongue guard, in that, the tongue guard was mounted to the side of the stone wheel which left an approximately 5/8 of an inch opening between the stone wheel.

The employer did not develop and implement a written Hazard Communication Program.
