29 CFR 1926.405(b)(2): All pull boxes, junction boxes, or fittings were not provided with covers and in energized installations each outlet box did not have a cover, faceplate, or fixture canopy:    a) 37 Trinity Rock Road on February 17, 2017, and at times prior, an outlet cover was missing for an outlet on the power pedestal marked "Global-3," exposing the receptacle to weather, and employees to electric shock.

29 CFR 1926.701(b): 1926.701(b):  All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement:    a) 37 Trinity Rock road - on February 17, 2017, and at times prior, 5 inches of exposed reinforcing steel ends were protruding from the surface of cured concrete, creating impalement hazards.
