The Strain relief on the electric cord was detached from the junction box. The junction box was not examined prior being used; on or about 2/17/17.    Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

The junction box was placed on the work floor; on or about 2/17/17.  b) Worksite, 17 Dune Rd., Quogue, NY- 2nd level: Employees were using a Relocatable Power Tap (stock #9900) to power charger, extension cords, and drill; on or about 2/17/17.   Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
