29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:     On or about February 16, 2017, and times prior thereto, employees stick welding on a 10-inch steel gas pipe inside an unprotected trench greater than 5 foot in depth were using a stepladder as a means of egress.

29 CFR 1926.652(b)(2): Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart.   On or about February 16, 2017 and times prior thereto, in a greater than 5 foot excavation, where employees were performing stick welding on a 10 inch steel gas pipe, the soil and rock deposits had not been adequately classified by a competent person as Stable Rock, Type A, Type B, or Type C.
