Production Area: The Landis machine used to thread rebar's had an unused opening that was not effectively closed by the electrical control which exposed employees to an electrical shock and burn hazard, on or about 3/30/2017.

Facility: On or about 3/30/2017 the employer did not record the following workplace injury and illness on the OSHA 300 Log for calendar year 2017. On or about 3/03/2017, a machine operator sustained an amputation to his right hand middle and index finger while using a machine to make threads on rebar.
