29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On February 21, 2017, the employer did not ensure that employees utilized energy control procedures on the equipment such as, but not limited to, the Mills and Lathes, prior to conducting activities such as, but not limited to, tool changes and job set up. The mills and lathes were not isolated from the electrical energy source prior to engaging in these activities. Employees were exposed to hazards from the rotating spindle and the moving tourette while all hazardous energy sources were not controlled prior to conducting work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)], are not implemented to protect employees from machine servicing hazards such as, but not limited to, tool changes and setup operations.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On February 21, 2017, the employer did not ensure that the employees operating the CNC Mills and Lathes were protected from the rotating spindle, the moving tourette and flying chips. The employees can open the door when the machines are running due to a safety interlock not being present or being manually disconnected.
