29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: Jobsite     An employee was exposed to falls greater than 11 feet to the ground below, while working on a steel awning attached to the side of a building, where fall protection had not been used.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Location: Jobsite  Employees were exposed to fall hazards using 8 foot aluminum step ladders, where not trained to recognize or avoid the hazards.
