29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse.  a) 5662 Mohawk St, Marcy NY, On or about 2/21/17: Employees were not provided with  safe and properly installed storage racking equipment in that, the all rack columns shall be anchored to the floor with anchor bolts capable of resisting the forces caused by the horizontal and vertical loads on the rack in  accordance with standard ANSI/RMI MH16.1  Specification for the Design, Testing and Utilization of Industrial Steel Storage Racks.  b) 5662 Mohawk St, Marcy NY, On or about 2/21/17: Employees were not provided with safe and properly installed storage racking equipment in that, safety clips were not installed on all beams in accordance with manufacturers label that is affixed to beams which states: "All beams must be installed with safety clips engaged. If clip is bent or missing, do not install beam. Call your USP distributor for replacement clips."
