29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or a near-miss incident:     A)    Phillips Feed & Pet Supply, Inc., located at 235 Norwest Drive, Clyde, OH 43410: On or about January 23, 2017, the employer did not provide refresher training in safe operation of sit-down powered industrial vehicles for the employee who was involved in an accident while driving one of the Crown forklifts (4500 Series) in the Staging Area. Employees were exposed to struck-by and caught-in-between hazards.

29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to travel with the load trailing whenever the load obstructed forward view:    A)    Phillips Feed & Pet Supply, Inc., located at 235 Norwest Drive, Clyde, OH 43410: On or about January 23, 2017, the employer did not ensure that drivers of powered industrial vehicles such as, but not limited to, the Crown forklifts traveled with the load trailing/in reverse while the forward view was obstructed by the load. Employees were exposed to struck-by and caught-in-between hazards.
