29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:     a) On or about 2/22/17, at the south end of second floor of the site, Grand Island, NY. A two-foot wide hole was not protected when a parked fork lift left its bucket 2 feet from the edge of the building. Employees were exposed to fall of 11 feet.     NO ABATEMENT CERTIFICATION REQUIRED
