Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Roof Level - Employees were observed on a low sloped roof while performing roofing operations and not protected from a fall hazard of 9 feet, 6 inches, on or about 02/23/2017.
