Hazardous conditions included: employees without fall protection working from unsecured platforms approximately 20 feet above concrete and asphalt; employee without fall protection working at the edge of the roof approximately 16 feet above the ground; working from inadequate job-made scaffolding; and improper use of stepladders and extension ladders, on or about 02/24/2017.   NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a)    North side of entrance overhang: Employee of Masters Builders Inc., without any type of fall protection, approximately 18-20 feet above ground level, was standing on top of an unsecured sheet of OSB resting on the forks of the Model B-66 Pettibone Telehandler while removing construction debris from the roof of the overhang, on or about 02/24/2017;  b)    North side of entrance overhang: Employees without any fall protection were working at heights greater than approximately 10 feet from an inadequate wood scaffold, fabricated from leftover scrap lumber, on or about 02/24/2017.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
