29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed: a) Wilson Sporting Goods Co. - Ada, Ohio: On or about February 28, 2017 the employer did not certify periodic inspections were being completed when employees engaged in die changes on the Franklin Roll Leaf Presses. Employees were exposed to amputation injuries in that adequate machine shutdown and isolation was not being followed when employees accessed hazard areas of the Franklin Roll Leaf Presses for die changes.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Wilson Sporting Goods Co.-Ada, Ohio: On or about February 28, 2017 the employer did not ensure that employees were protected from the unexpected energization of the Franklin Roll Leaf Stamping Presses during servicing. Employees are exposed to amputation hazards in that the Franklin Roll Leaf Stamping Presses are not locked out prior to changing dies.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: a) Wilson Sporting Goods Co. - Ada, Ohio: On or about February 28, 2017 the employer did not ensure that employees who performed die changes on the Franklin Roll Leaf Presses were trained in the skills required for the safe application, usage, methods, and means to isolate and control energy sources. Employees accessing the hazard areas of the press were not trained to lockout the machine to protect against amputation injuries.
