29 CFR 1910.213(d)(1): Each circular crosscut table saw was not guarded by a hood which shall meet all the requirements of paragraph (c)(1) of this section for hoods for circular ripsaws:  a) Thermal Craft Insulation, at 5186 North 109th Street, Longmont, CO 80504:   On and before November 21, 2016, the employer did not ensure that the table saw used in the shop was guarded.  A table saw used to cut wood for a box did not include an automatically adjusting hood to completely enclose that portion of the saw above the table and above the material being cut.  This condition exposed employees to amputation hazards.
