29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   On 03/01/2017, Precision Prototyping and Manufacturing, Inc., located at 10740 Midwest Industrial Blvd., St. Louis, MO, had not established a program consisting of energy control procedures, employee training and periodic inspections.  There were pieces of equipment with more than one energy source (i.e., the Durma Shear with electrics and potential energy, the Water Jet with electric, pneumatics and pressurized water).

This 6 inch bench grinder, Model: JBG-6A, S/N: 030601005, 1.5 hp, had an rpm speed of 3450.
