29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   (a) 303 Bryant Street Mechanicsburg, Pa 17050 -The employer did not ensure that employees standing on the porch roof installing sheet metal on the soffit were protected from falls, with guard rails, safety nets or personal fall protection, thereby exposing employees to a fall hazard of approximately 16 feet, on or about March 2, 2017.      No abatement certification or documentation is required.

29 CFR 1926.1053(b)(4):  Ladders were used for purposes other than the purpose for which they were designed:  (a) 303 Bryant Street, Mechanicsburg PA  - The employee was using a 6 foot green fiberglass step ladder placed it on the porch roof in the closed position leaning up against the wall to access the soffit of the house, thereby exposing the employee to injuries from ladder deflection and a fall of approximately 16 feet, on or about March 2, 2017.  No abatement certification or documentation required.
