29 CFR 1910.28(b)(11)(ii): Stairways. The employer did not ensure each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail systems and handrails per Table D-2; stair width less than 44 inches (1.1 m), and two open sides required to have one stair rail system on each open side.   (a) Brick Strapping Area: On or about March 7, 2017 the fixed stairs that lead up to the brick strapping area did not have standard hand rails on both sides.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt.  (a) Brick Strapping: On or about March 2, 2017 operator at the brick strapping machine was exposed to unguarded horizontal belts.
