29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   At the construction site located at 209 N State 291 in Liberty, Missouri.  Employees were exposed to a fall hazard when performing roofing operations.  Employees were working at the commercial structure at an elevated height of approximately 21-26 feet above the concrete surface.   Employees were not provided fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   At the construction site located at 209 N State 291 in Liberty, Missouri.   Employees were exposed to a fall hazard while using a ladder.  The ladder did not extend 3 feet above the landing surface of the roof to which they gained access.
