29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a) Toledo Industrial Center, DBA Lithium Innovations Company, LLC located at 1303 Campbell Street in Toledo, Ohio; On or about March 2, 2017 the employer did not ensure employees were protected from electrical hazards in that the electrical wire studs supplying electricity (480 volts) to heater bands located on the outside of the mold were unprotected from contact.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a) Toledo Industrial Center, DBA Lithium Innovations Company, LLC located at 1303 Campbell Street in Toledo, Ohio; On or about March 2, 2017 the employer did not develop and maintain hazard communication program at the workplace. Employees were exposed to hazards associated with, but not limited to lithium foil and argon gas.
