29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) Southmoore Crossing, Bath, Worksite - On or about March 3, 2017, employees were working on a steeply pitched residential roof, approximately 20 feet above grade, without utilizing any means of fall protection.    b) Southmoore Crossing, Bath, Worksite - On or about March 3, 2017, employees were working in a structure with unguarded window openings, approximately 16 feet above grade, without utilizing any means of fall protection.    c) Southmoore Crossing, Bath, Worksite - On or about March 3, 2017, employees were working in a structure with unguarded landings and stairs, approximately 9 feet above a lower level, without utilizing any means of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Southmoore Crossing, Bath, Worksite - On or about March 3, 2017, employees were performing frame carpentry work where multiple fall hazards were present and the employer had not provided fall protection training for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the related methods of minimizing these hazards.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge:    a) Southmoore Crossing, Bath, Worksite - On or about March 3, 2017, employees were working on the second floor of a residential home project, while using a stairway rising more than 30 inches above a lower level to access the upper floor which did not have a stair rail system along its unprotected side.
