29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: 104-112 Harding Street, Worcester Ma: On or about March 3, 2017 an employee was utilizing a Miller 6 foot shock absorbing lanyard which had orange electrical tape covering the deceleration device cover not allowing for it to be thoroughly inspected for wear, damage or other deterioration prior to use.
