29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    (a) Transit Mix Concrete & Materials Company: On or about March 3, 2017, and at times prior, employees were exposed to electrical hazards due to a flexible cord on the batch mixing drum not connected to a device in a manner which prevented tension being transmitted to joints or terminal screws.  Transmission of tension contributed to excessive wear on the inner and outer insulation of the cord, resulting in employee exposure to non-insulated copper wiring.    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
