29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: At a commercial construction project, employees were installing sun shades from an unsecured pallet platform elevated by a forklift and not authorized by the forklift manufacturer exposing them to fall hazards.

29 CFR 1926.602(d): The employer did not ensure that each employee operating a powered industrial vehicle had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178. and as referenced by the note contained in 1926.602(d). At a commercial construction project, the employer did not ensure that the operator of a Hyster H50FT forklift was trained prior to allowing the employee to operate the equipment exposing employees to crush-by, struck-by hazards and fall hazards.
