29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied.  Worksite: The employer did not ensure that the entire scaffold platform from which the employee was required to work in an elevated position was secured to the forks of the all-terrain forklift that was used to support the platform.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Worksite: The employer did not ensure that the employee who was required to work from the elevated scaffold platform at a height above 10 feet was protected from falling by the use of appropriate fall protection systems/devices.

29 CFR 1926.602(c)(1)(vi): All industrial trucks in use shall meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standard Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. Section 603 General (E): When leaving a powered industrial truck unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shut off, brakes set, key or connected plug removed. Block wheels if truck is parked on an incline.  Worksite: The employer did not ensure that the operator of the all-terrain forklift was attended at all times, while the employee was working from the supported scaffold platform.
