29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  On or about August 4, 2017, the employer failed to submit certification of corrective action to OSHA that the following hazards have been abated:  a: Citation 1, item 1/a, On or about March 7, 2017 at 609 Baymist Drive, Loganville, Georgia, an employee performing roofing work was exposed to a fall hazard of approximately 20.7-feet when he was not provided with proper fall protection equipment from his employer who required employees to purchase their own fall protection equipment.   b: Citation 1, item 1/b, On or about March 7, 2017 at 609 Baymist Drive, Loganville, Georgia, an employee performing roofing work was exposed to a fall hazard of approximately 20.7-feet when he used a rock climbing harness and attached a pelican hook, carabiner, and rope at the front of his waist.

29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On or about August 4, 2017, the employer failed to submit documentation and certification of corrective action to OSHA that the following hazards have been abated:  a: Citation 1, Item 1/a, On or about March 7, 2017, at 609 Baymist Drive, Loganville, Georgia, an employee performing roofing work was exposed to a fall hazard of approximately 20.7-feet when he was not provided with proper fall protection equipment from his employer who required employees to purchase their own fall protection equipment.  b: Citation 1, Item 1/b, On or about March 7, 2017, at 609 Baymist Drive, Loganville, Georgia, an employee performing roofing work was exposed to a fall hazard of approximately 20.7-feet when he used a rock climbing harness and attached a pelican hook, carabiner, and rope at the front of his waist.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
