29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. Employees installing roof shingles on a 6:12 pitch roof at elevations of 6 feet or more above the ground are not being protected from fall hazards by a fall protection system.      The employer fails to assure effective protection against falls as required by the standard, as demonstrated on March 8, 2017, when employees installing roof shingles on a 6:12 pitch roof at 32 Sandra Drive, Lot 17, Ormond Beach, FL 32176, were exposed 10 feet fall hazard above the ground in that they did not have a fall protection system installed.     Bert Faircloth Roofing, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1144097, citation number 01, item number 001 and was affirmed as a final order on June 15, 2016, with respect to a workplace located at 3356 Newbliss Court, Ormond Beach, FL 32173.    Bert Faircloth Roofing, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1133294, citation number 01, item number 002 and was affirmed as a final order on April 22, 2016, with respect to a workplace located at 1409 Wildrose Lane, Daytona Beach, FL 32117.
