29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year.  a) At the facility, the OSHA Form 300 for calendar year 2016 was not accurate and complete.  The injury description was not completed for cases #4 and #7;  case #4 is checked as a job transfer case under column I, but 95 days away from work are listed on column K, instead of transfer under L; and a total of 3 cases with injury are noted but injury types in (M) reflect zero for all types.
