29 CFR 1910.178(o)(1): Unstable or unsafely arranged loads were handled.  Caution was not exercised when handling off-center loads which could not be centered:    ESTABLISHMENT:  An unstable or unsafely arranged load by placing a dolly under the load was handled with the forks of the Hyster forklift truck without appropriately securing the load and/or exercising a caution.    On or about 03/08/17, the load while being lifted tipped and fell onto an employee's leg, and the employee sustained sprains to both ankles.
