29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole:   a. Building 11, employees were exposed to a 9 foot fall hazard, in that, access from the second floor to the third floor was through a floor hole which did not have a gate or railings installed in such a manner as to offset the fall into the floor hole, on or about 03/08/2017.
