29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. On or about March 8, 2017, on the 5:12 pitch roof trusses, framing employees installing permanent truss bracing were exposed to fall hazards from heights of 10 to 21 feet from eave edges and through the open trusses.

On or about March 8, 2017, on the 5:12 pitch roof trusses, employees exposed to 10 to 21 feet fall hazards had not been trained to properly install anchor systems for personal fall protection equipment.
