29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest.  1) 2925 Headland Drive, Atlanta, GA: On or about March 8, 2017, employees were working on a flat roof without fall protection; installing a new roof on the commercial building and was exposed to a fall hazard of approximately 16 feet high.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  1) 2925 Headland Drive, Atlanta, GA: On or about March 8, 2017, employees were using a 18 foot ladder to access and egress the upper level of the roof.  The ladder did not extend at least 3 feet above the landing and/or tied to the building to prevent displacement.
