On or about March 9, 2017, on the south west side of a 5/12 pitch roof of a residential condominium, an employee installing asphalt shingles while using a pneumatic nail gun did not wear eye protection, exposing the employee to the hazard of being struck in the eye or face by flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a.  On or about March 9, 2017, on the south side of a 5/12 pitch roof of a residential condominium, seven employees were installing asphalt shingles and were not protected from a 25 foot fall hazard by the use of a personal fall protection system.    Holloway Roofing Unlimited, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d)(16)(iii), which was contained in OSHA inspection number 1100936, citation number 02, item number 01 and was affirmed as a final order on December 22, 2016, with respect to a workplace located at 102 Quince Ave., Niceville, FL 32578.    Holloway Roofing Unlimited, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 472548, citation number 01, item number 02 and was affirmed as a final order on August 15, 2012, with respect to a workplace located at 422 Bally Way, Niceville, FL 32578.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a.  On or about March 9, 2017 on a jobsite of a commercial building, the employer did not ensure that an employee was certified to operate a company rented JLG, 6000 lbs. forklift, exposing employees to struck-by and crushing hazards.  In the alternative:  1926.602(d)  Powered industrial truck operator training.  Note:  The requirements applicable to construction work under this paragraph are identical to those set forth at sub-section 1910.178(l) of this chapter.
