29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees:    a)  On or about March 13, 2017, employees were exposed to fire and explosion hazards where an underground gas line was not adequately supported during excavation operations. Lack of support for the gas line caused it to rupture.
