29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system.  Jobsite: On or about March 9, 2017 employees were exposed to falls of approximately 19 feet while working from a ladder jack scaffold without the use of personal fall arrest systems.

Jobsite: On or about March 9 2017 the employer did not assure that each employee working on ladder jack scaffolding, and exposed to a fall of approximately 19 feet, was trained by a competent person qualified in the areas specified in 29 CFR 1926 (a)(2)(I) through (viii).
