29 CFR 1917.45(e)(2): Crane hooks were used without safety latches or other means to prevent accidental load disengagement:  (a)  Alyeska Seafoods, Crane #1:  On or about March 8, 2017 and at times prior thereto, the hook latch was inoperative.  This condition exposed employees to struck by hazards in the event of disengagement.  (b)  Alyeska Seafoods, Crane #3, F/V Arctic Mariner:  On or about March 8, 2017 and at times prior thereto, the hook latch was inoperative.  This condition exposed employees to struck by hazards in the event of disengagement.

29 CFR 1917.93(b): Head protection equipment did not comply with American National Standard Institute ANSI Z-89.1-1986 requirements:  Alyeska Seafoods, F/V Arctic Mariner:  On or about March 8, 2017 and at times prior thereto, employees engaged in marine cargo handling where there was a potential for injury to the head from falling objects, utilized head protection that did not comply with the referenced ANSI standard.  This condition exposed employees to struck-by hazards.
