29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    A.) On or about Friday, February 24, 2017 at 38-42 11th Street, Long Island City, NY 11101    Employees working approximately twenty-five (25) feet above the lower level performing a formwork operation were not prevented from a free fall distance of more than six (6) feet.  An employee fell approximately 25 feet to the lower level.    ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:    A.) On or about Friday, February 24, 2017 at 38-42 11st Street, Long Island City, NY 11101    The employer failed to ensure personal fall arrest system components used by employees were inspected prior to each use for wear, damage and deterioration. Lanyards used by employees were observed to be torn.    ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
