On or about Friday, February 24, 2017 at 38-42 11th Street, Long Island City, NY 11101 Employees working approximately twenty-five (25) feet above the lower level performing a formwork operation were not prevented from a free fall distance of more than six (6) feet. An employee fell approximately 25 feet to the lower level. ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

On or about Friday, February 24, 2017 at 38-42 11st Street, Long Island City, NY 11101 The employer failed to ensure personal fall arrest system components used by employees were inspected prior to each use for wear, damage and deterioration. Lanyards used by employees were observed to be torn. ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
