29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.   On or about March 15, 2017, employer did not ensure that employee(s) working on the roof were provided with and required to use adequate fall protection. Employees were exposed to fall hazards from and elevation exceeding 6 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about May 15, 2017 the employer did not ensure that employee working on a low pitch roof were provided with fall protection training necessary for hazard recognition and safe work practice. Employees were exposed to fall hazards exceeding 6 feet.
