29 CFR 1910.134(i)(6): The employer did not have a means of ensuring that carbon monoxide levels in the breathing air did not exceed 10 ppm:  On or about 15 March 2017, an employee conducting sandblasting operations was using an air compressor to supply his breathing air where the employer did not ensure that it was fitted with a carbon monoxide alarm, or other similar effective means of ensuring that carbon monoxide levels did not exceed 10ppm, exposing the employee to health hazards.

29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146:  On or about 15 March 2017, in the sandblasting area of the shop, an employee was sandblasting the inside of a pneumatic trailer, which is a permit required confined space, before he was provided with confined space training by the employer, exposing him to hazards such as but not limited to inhalation hazards.

29 CFR 1910.146(i)(4): The employer did not ensure that each attendant remained outside the permit space during entry apparitions until relieved by another attendant:  On or about 15 March 2017, in the sandblasting area of the shop, the attendant who was stationed outside of the employee's workspace, left that location while the employee was sandblasting the inside of a pneumatic trailer which was a permit required confined space, exposing the employee to the delay of rescue and/or assistance in the event of an emergency.

29 CFR 1910.134(c)(1): The employer did not establish and implement a written respiratory protection program with worksite-specific procedures in any workplace where respirators were necessary to protect the health of the employee or whenever respirators were required by the employer:  On or about March 15, 2017, the employer did not ensure that each of the following provisions were established and implemented in the respiratory protection program where employees are required to wear respirators:   1910.134(c)(1)(i)-Procedures for selecting respirators for use in the workplace;  1910.134(c)(1)(ii)-Medical evaluations of employees required to use respirators;  1910.134(c)(1)(iii)-Fit testing procedures for tight-fitting respirators;  1910.134(c)(1)(iv)-Procedures for proper use of respirators in routine and reasonably foreseeable emergency situations; 1910.134(c)(1)(v)-Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding and otherwise maintaining respirators;  1910.134(c)(1)(vi)-Procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators; 1910.134(c)(1)(vii)-Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations;  1910.134(c)(1)(viii)-Training of employees in the proper use of respirators, including putting on and removing the, any limitations on their use, and their maintenance; and 1910.134(c)(1)(xi)-Procedures for regularly evaluating the effectiveness of the program.
