29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    (a) 375 Roma Jean Parkway, Streamwood, IL - The employer's Personal Protective Equipment (PPE) assessment did not address the PPE necessary for grinding and mechanical cutting, which present struck-by hazards to the face. On or about March 13, 2017, an employee was using a Makita 9557PB angle grinder equipped with a Performax 4.5 inch metal cutting wheel to remove a bearing cap when the cutting wheel failed and a piece of the wheel struck the employee on the forehead.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.133(a)(1): Protective face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  (a) In the yard behind the main building - On or about March 13, 2017, an employee was using a Makita 9557PB angle grinder equipped with a Performax 4.5 inch metal cutting wheel to remove a bearing cap when the cutting wheel failed and a piece of the wheel struck the employee on the forehead. The employee was not provided with or required to wear a face shield while operating the grinder.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

The guard on the grinder did not cover the spindle end and flange projections. 29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:  (a) In the yard behind the main building - On or about March 13, 2017 an employee was using a Makita 9557 PB angle grinder equipped with a Performax 4.5 inch metal cutting wheel to remove the bearing cap when the cutting wheel failed and a piece of the wheel struck the employee on the forehead. The guard on the grinder was not properly adjusted to ensure that the enclosed side of the guard pointed toward the operator.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
