Worksite located at 1615 Ashton Circle, Liberty, Missouri- Employees were observed tearing off existing roofing materials and were not wearing any means of eye protection to protect against flying particles when the debris comes off the roof.

Worksite located at 1615 Ashton Circle, Liberty, Missouri- The gas container at the jobsite being used by the employer was not a DOT approved container with spring-closing lid and spout cover, a means to relieve internal pressure, and flash-arresting screen.
