29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Residential Golf Community: Procedures were not utilized to ensure the snow blowers were de-energized when workers were required to unclog snow from the chute.     On 2-9-17  A worker suffered an amputation injury while removing clogged snow from the blower while the auger was still in operation.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:    Residential Golf Community: An accident that took place on 2-9-17 involving an employee's finger amputations, was not reported to OSHA within 24 hours.
