29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports: a) On or about March 20, 2017, employees were exposed to the hazard of falling approximately 18 feet while working from scaffolding that was not fully planked. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(e)(1): Safe access was not provided to scaffold platforms when the platforms were more than 2 feet (0.6 m) above or below a point of access: a) On or about March 20, 2017, employees were exposed to the hazard of falling approximately 20 feet where safe access between a roof deck/top of a masonry wall and the landing surface of a scaffold platform was not provided. The break in elevation between the roof deck/top of masonry wall was about 3 feet at an exposure height of about 20 feet. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
