29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:     (a) Xylem Designs at 309 Hickory St, Fort Collins, CO: On and before March 17, 2017, the employer did not ensure that personal protective equipment or clothing for extremities was provided and used whenever hazards capable of causing injury and impairment were encountered. An employee, who was conducting spray activities in a Brinks paint booth with lacquer and stain dyes, wore shorts, leaving portions of his legs uncovered. This condition exposed the employee to category 2 skin corrosion/irritant chemical hazard.      Abatement Note: If such hazards are present, or likely to be present, the employer shall: Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment; communicate selection decisions to each affected employee; and, select PPE that properly fits each affected employee. Note: Non-mandatory Appendix B contains an example of procedures that would comply with the requirement for a hazard assessment.

Plastic secondary chemical squeeze bottles  and plastic secondary chemical containers were being stored in the northwestern corner of the work floor. The chemicals were identified with handwritten labels and a color code system which did not provide at least general information regarding the hazards of the chemicals or the information specified under paragraphs (f)(1)(i) through (v) of this section. The chemicals in the secondary containers included but were not limited to lacquer thinner, vinegar, paint thinner, and isopropyl alcohol.    (b) Xylem Designs at 309 Hickory St, Fort Collins, CO: On and before March 17, 2017, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemicals contained within. Plastic secondary chemicals containers were being stored in the spray booth. The chemicals were identified with handwritten labels which did not provide information specified under paragraphs (f)(1)(i) through (v) of this section. The chemicals in the secondary containers included but were not limited to General Finish Dye Stains with lacquer.
