29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall:  (a) On or about 3/15/2017 - at Section/Building 3, 936 22nd Ave E, Tuscaloosa, Al, additional protection from falling hand tools, debris, and other small objects was not provided for an employee who was applying exterior insulation and finish system (Dryvit) from a scaffold platform while another employee was also applying Dryvit from a scaffold platform above him.

(b) On or about 3/15/2017 - at Section/Building 3, 936 22nd Ave E, Tuscaloosa, Al, employees applying exterior insulation and finish system (Dryvit) on a wall were working from scaffold platforms that were not fully planked.

Crossbraces were used as a means of access:    (a) On or about 3/15/2017 - at Section/Building 1, 936 22nd Ave E, Tuscaloosa, Al, a ladder or other means of access was not provided for an employee that was applying exterior insulation and finish system (Dryvit) from a fabricated frame scaffold approximately 19 feet above the ground. (b) On or about 3/15/2017 - at Section/Building 3, 936 22nd Ave E, Tuscaloosa, Al, employees were applying exterior insulation and finish system (Dryvit) from a fabricated frame scaffold that was not fully braced across all sections due to diagonal braces missing on the side adjacent to the wall and the scaffold sections were not secured together laterally.
