29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.  a) On or about March 7, 2017 or prior there to, on NMW-C, Vertical 5-axis Mill:   DMG Mori New West employees did not utilize energy control procedures before servicing the control mechanism of the Mill.   Power was disengaged prior to  maintenance activities by turning a rotational power switch without utilizing a lock out mechanism.  NOTE:   Abatement certification is required for this item.

29 CFR 1910.147(f)(2)(i): Outside servicing personnel were engaged in activities covered by the scope and application of this standard, and the on-site employer and the outside employer did not inform each other of their respective lockout or tagout procedures.  a) On or about March 7, 2017 or prior there to, on NMW-C, Vertical 5-axis Mill:   The outside servicing employer, DMG Mori New West,  did not review energy control procedures with Quality Machine Products before servicing the control mechanism of the Mill.   Power was disengaged by the contractor prior to maintenance activities by turning a rotational power switch without a lock out mechanism.  NOTE:   Abatement certification is required for this item.
