29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about 10/1/2015 at 3501 Kumho Parkway Macon GA in the loading bay; employees were exposed to shock hazards where the Maxx Pro 42inch industrial floor fan provided by the host employer was in use. Due to the yellow extension cord missing the grounding pin the outer casing was electrified. Additionally, with the potential of water collecting around the fan from powered industrial trucks entering from outside, the hazard is created.
